Welcome to our practice! We hope that the following information will be helpful to you. We respect your time and we would like to make your visit to our office as efficient as possible.
Location:
We are located on the north side of Memorial Hermann Hospital of the Woodlands in Medical Office Building III.
Office Policy:
Please arrive 15 minutes prior to appointment time to confirm that all information is obtained. Unfortunately, due to the busy nature of our practice if you arrive 15 minutes after your appointment time you may be asked to reschedule.
Cancellation:
Our office sees patients by appointment only; if you are unable to keep your appointment please notify us within 24 hours. If an appointment is not kept or cancelled without 24 hour notice there will be a $25 fee.
Financial Responsibility:
Payment is due at time services are rendered unless other arrangements have been made in advance. Please call our billing department at (281) 292-5774 x102 for any questions. Accepted methods of payment are Visa, MasterCard, Discover, check, and cash.
Medical Insurance:
Please bring your current insurance card and picture ID. This will assist us to file for reimbursement from your insurance company on your behalf. Your insurance may not cover the full cost of your studies or physician services. Regardless of insurance, payment remains the patient responsibility. Any questions regarding insurance coverage please call your insurance company first, if questions still remain feel free to call our office.
Medical Records:
If you are transferring care to us during your pregnancy your records from your previous doctor are required. All others may request medical records prior to appointment using the release form enclosed or wait to complete the form at your appointment.
